    • 1). Download and print the application for child support from the Florida Department of Revenue's website (see Resources). The form is available as a PDF file that can be saved to your computer.

    • 2). Complete the child support application. The application asks for details such as the Social Security number and date of birth of the child and both parents. If you are unable to answer any of the questions, write a brief explanation on a separate sheet of paper explaining why you are unable to do so.

    • 3). Review your application for accuracy. Correct any errors you discover. Be sure to select the services for which you would like to apply. If you would like assistance with establishing paternity and collecting child support, you need to select "Full Services." If you would just like assistance in locating the other parent, select "Location-Only Services."

    • 4). Gather the necessary documents to file for child support. You need a copy of the child's birth certificate and copies of any payment records concerning the child. You also need to provide any proof of an agreement between you and the other parent.

    • 5). Mail the application and the documents to the child support office for Pinellas County. The address is 19361 U.S. Highway 19 North; Clearwater, FL 33764-3102. Review the forms before mailing to avoid a delay in processing your child support application.
