    • 1). Gather all of your documentation. For example, you will need birth certificates of all household members, social security cards, check stubs, bank statements and proof of address.

    • 2). Visit your local Department of Social Services to complete an application for Medicaid. The name for this department will vary by state. For example, in Georgia the agency is referred to as the Department of Family & Children's Services.

    • 3). Submit your completed application and documents from Step 1 to the Social Services representative.

    • 4). Download and print out a medicaid application from your state's Department Of Social Services website if you don't want to visit the office in person. Mail the completed application, along with copies of your documents, to the the address listed on the application.

    • 5). Apply online for Medicaid if your state agency has an online application portal. You will still have to submit your documentation to the agency, according to the instructions provided in the online application.
