How to Apply for Guardianship Subsidy in Arizona
- 1). Obtain an "Application for Benefits (FA-001)" form at any Family Assistance Administration (FAA) office to apply for Temporary Cash Assistance for Needy Families. The program is usually called TANF/Cash Assistance and the application is also the necessary first step to apply for Food Stamps in Arizona and to enroll the child in a medical health insurance program named the "Arizona Health Care Cost Containment System (AHCCCS)."
- 2). Complete the form and return it to the nearest FAA Office or apply online at www.HealthEArizona.org.
- 3). Wait for approval or denial of your application for TANF/Cash Assistance, Food Stamps and AHCCCS medical insurance.
- 4). Send a copy of the approval or denial letter from the Family Assistance Administration to Child Protective Services which is a branch of the Arizona Department of Economic Security (DES).
